Jamestown School Lawn Athletics
In order to participate in any sport, ALL Athletes must upload all 4 completed forms into the TeamSnap registration site:
1. Assumption of Risk form (Good for all 3 middle school years)
2. Concussion Awareness Form (needed for each sport)
3. Current RI School Physical (within 1 year)
4. Jamestown School Athletic Rules and Responsibilities (completed annually)
