INFORMATION FOR PARTICIPANTS:
-There is no out of pocket cost. Insurance information will be collected, but no one will be turned away for lack of insurance.
-This clinic is open to the community. No need to be affiliated with the school.
-Please bring COVID-19 vaccine records or cards.
-Please ask for proof of a booster before leaving the clinic. The school department does not have access to all of these records.
-There is limited supply for walk-ins so please register so enough vaccines are supplied.
-Only the bivalent booster will be offered at this clinic, not the primary vaccine series.
-Participants should stay for a 15-minute observation after vaccination.
