New Student Registration
FOR ALL NEW INCOMING STUDENTS
Please complete the Registration Form and the Release of Records Form as soon as you know you will be moving to Jamestown.

Required for Registration
- Proof of Residency in Jamestown (copy of utility bill, lease, etc.)
- Copy of Birth Certificate
- Home Language Survey
- Transportation Form, found on the Transportation Page. Please read the Transportation Letter prior to filling out form.
Required Medical information - must be submitted to the school nurse prior to starting school:
- Current physical examination – within the past twelve months (Click here for form to request from physician).
- Current list of immunizations - see below messages for requirements.

Required for 7th grade/8th grade Accelerated Math:
- Students who wish to be considered for accelerated math in grade 7 and grade 8 must complete a math assessment prior to the beginning of the school year. Students can schedule this assessment on the following days and times: Tuesday and Wednesday 8:30-11:30. If these days and times do not work please call the office for an alternate time.
Required Music Form for Students in Grades 5-8
INTEGRATED PRESCHOOL INFORMATION
Please complete the Application Form by February 28. All families will be notified by the second week of March. There is limited space in the program. If applications exceed the number of students allowed, we will hold a random lottery for all new students.
AM Preschool - Must be 3 years old by Sept 1st - The program runs Monday through Thursday from 8:05 -11:00am.
PM Preschool - Must be 4 years old by Sept 1st - The program runs Monday - Friday 10:15 - 2:40pm.
FOR ALL HIGH SCHOOL STUDENTS
NARRAGANSETT HIGH SCHOOL REGISTRATION
Bring the Narragansett High School Registration Form and proof of residency to Melrose Office prior to registering at high school to obtain a required Verification of Residency Voucher from Jamestown.
NORTH KINGSTOWN HIGH SCHOOL REGISTRATION
Bring the North Kingstown High School Registration Form and proof of residency to Melrose Office prior to registering at high school to obtain a required Verification of Residency Voucher from Jamestown.
