Health Reminders:
 
1. Please submit a copy of yearly physical exams, updated immunizations, and dentist/eye doctor/specialist appointment dates so that all health records are complete. Physical exam forms are required for students entering Kindergarten, 7th grade, and 12th grade and any student new to the school or participating in a school sport. 
 
2. Please see links for medication policies, forms, and immunization requirements. Students entering PK, K, 7th, and 8th, please see memos. Students in 1st-6th grades should have at least all vaccinations listed on the kindergarten reminder. Click here for RIDOH immunization page.
 

3. If you would like your student to have the option of receiving an over-the-counter medication at school, we will need the OTC Medication Form filled out by a physician and parent for any OTC medication given at school (Tylenol, Motrin, Tums, cough drops, etc.). Medication forms are good for the current school year only.

 

4. All medications (including cough drops, Tums, etc.) must be handed to the nurse by the parent/guardian (not sent to school with the student) and must be kept in the nursing office unless we have written permission by the student's health care provider.

 

5. A student should not come to school if they have experienced fever 100F or higher, vomiting, or diarrhea within 24 hours (if medication has been used, restart the clock).
